This form is used to submit information for the NJIT Residence Life Community Involvement points incentive program. You should complete this only if you're planning on participating in room selection for the Fall 2013 semester.

To complete the below form, please fill out the relevant fields for the sections that best describe your activities. For you to receive points for that section, the advisor named will need to confirm your involvement. Please ensure that the e-mail address is correct and active, as your advisor will receive an automated email after submission. It is your responsibility to provide accurate information and to have your advisor confirm your involvement.
