Fill out the Application

Bring the completed application and a documentation of income* to the SSSP office at GITC 3420
* Documentation of income is to be provided by each applicant. Acceptable proofs of income include:

  • A signed statement from the individual (if independent) or individual’s parent or legal guardian (Form is available)
  • Verification from another government agency
  • A signed financial aid application; or
  • A signed United States or Puerto Rico income tax return

First Generation Criteria:
Did your natural or adopted mother have a baccalaureate degree?
Did your natural or adopted father have a baccalaureate degree?
Disability Criteria:
Do you have documented disabilities?
If yes, is the information filed with the Center for Counseling and Psychological Services?
Income Criteria:
For Financial Aid purposes, are you considered a (check one)
