Medical Exemption
Requires a letter from a healthcare provider (physician or nurse practitioner) and must state the medical contraindication to receiving the specific vaccine. The letter must be signed and dated by the provider and must contain the providers credentials. Submit letters for medical exemptions to NJIT.Medicatconnect.com under "Medical Exemption" from the drop down menu on the home page.
Religious Exemption
Students requesting a religious exemption are to submit a letter stating how the specific vaccine conflicts with a sincerely held religious belief or practice.
