Student Pre-entrance Health Requirements
In the interest of maintaining a safe and healthy campus, all full and part-time students, are required to complete the Pre-Entrance Health Requirements packet.

Please read all information on this site carefully to make sure you understand all requirements.
New students (all levels and statuses) must submit immunization records, complete tuberculosis screening, and review meningococcal education by the posted deadlines.
Re-admitted and new graduate students (continuing NJIT undergraduates) may need to provide updated information not previously required or submitted.
All health documents (immunization records and lab reports) must be submitted through the Medicat Portal. Tuberculosis screening and meningococcal education are also completed in the Medicat Portal.

Immunization Record:
Note: If you were vaccinated in NJ your immunization records may already be in our system. Check your file by logging into the Medicat portal and choosing IMMUNIZATIONS from the main menu. Immunizations on file are displayed in green on the far right of the page. Those not on file, or those which are not completed, are noted in red.
Acceptable Immunization Forms
- NJIT Immunization Record Form; or
- An official immunization record from a healthcare provider (signed, dated, and stamped); or
- Record from a previous school, state immunization database, military branch, or employer; or
- Lab report showing immunity (the actual laboratory results must be submitted for review and acceptance)
Accessing Your Immunization Records
Individuals immunized in New Jersey (as well as Alaska, Idaho, Maine, Minnesota, Missouri, North Dakota, Utah, and Wyoming) can download a PDF copy of their immunization records through the Docket app.
Individuals immunized in other states can access their records through their state’s immunization database.
Invalid Documentation
Documents that have been modified (e.g., overwritten, crossed out, or whited out) are not considered valid.
Documents without an official English translation are also not considered valid.
Tuberculosis Screening and Testing
All students must complete the tuberculosis screening form in the Medicat portal. Once in the portal choose FORMS from the main menu and select TUBERCULOSIS QUESTIONNAIRE. Testing is required if the answer to any question is "Yes." A blood test or skin test is acceptable.
Meningococcal Disease Education
All students must read the MENINGOCOCCAL DISEASE EDUCATION FORMS located in the Medicat portal. Once in the portal choose FORMS from the main menu and select MENINGOCOCCAL DISEASE EDUCATION FORMS. Complete the ATTESTATION FORM after reading through the meningococcal disease information

If you need vaccinations or a physical exam and are seeking affordable care, please visit the New Jersey Department of Health website for available options.
You may also consider the Mary Eliza Mahoney Health Center in Newark, which offers vaccinations and a range of other medical services.
