Step Three: New Member Forms

If you are going through the educational process for a fraternity or sorority organization, there are a couple forms the Office of Fraternity & Sorority Life requires you to complete. Please see the information below.

For Future Members

All Councils New Member and Intake Forms

Students must complete registration for their respective council prior to the invitation to join their organization, if you have not completed a recruitment form for your respective chapter return to step two and complete it now, then come back here.

All students must complete the New Member Registration Form within 72 hours of starting their process.

Each semester students joining a Greek Lettered Organization will need to attend New Member Education sessions hosted by the Office of Fraternity and Sorority Life.

Fall Sessions:

  • Friday October 10th @11:30am Welcome Fair - Campus Center Atrium
  • Wednesday October 15th @ 3:00pm Academic Workshop - Virtual
  • Friday October 24th @11:30am Risk Management - TBD
  • Wednesday October 29th @3:00pm Title IX/Bystander Training - TBD

New Members/Candidates must also complete an online module known as Tightrope by November 23, 2025.

Once forms are completed, information for completing online training modules will be sent out to each new member. Please be sure to read the instructions attached to that email.
