OGI requires students to submit all I-20 requests and applications electronically using the OGI submission request form below.

 OGI APPLICATION & DOCUMENT REQUEST/SUBMISSION FORM 

Submit the Google form using the link above.

Be sure to upload the corresponding application form (found below) and all required documentation in 1 PDF, as needed.

 

Once processed, students will receive the new Form I-20, signed by an OGI adviser, via email. You must print and sign in ink your I-20.

FULL-TIME CERTIFICATION

Adviser Form For Reduced Course Load Use this form if you are an international student (Bachelor, Masters) requesting a reduced course load for academic reasons.
Reduced Course Load for Medical
Reasons Request Form		 Use this form if you are an international student (Bachelor, Masters) requesting a reduced course load for medical reasons.  Medical documentation from a U.S. based doctor is required.

 

EMPLOYMENT

J-1 On campus authorization  Use this Form if you are a J-1 student applying for a job on-campus.
F-1 Curricular Practical Training Request (No form required)

You must follow CDS' application procedure through Handshake. 

 

F-1 Optional Practical Training (OPT) Request Form

  • Log in using your UCID and password.
  • Click on "F-1 Practical Training" on the left-side menu
  • Click on OPT Request

LEAVING NJIT

Leave of Absence form for Undergraduate and Master's students

Use this form if you are an undergraduate or master's student who will take a leave of absence from NJIT next semester.

(Doctoral students interested in a Leave of Absence must contact the Office of Graduate Studies). 

 

PROGRAM EXTENSION

F-1 Extension Form

Use this form to apply for an extension of your end date before your current Form I-20 expires.

 
J-1 Extension Form

Use this form to apply for an extension of your end date before your current Form DS-2019 expires.

 
Financial statement 2025-2026 Use this form to show you have sufficient funding to cover your expenses.

 

MISCELLANEOUS 

Application for Form I-20

Use this form if you are an F-1 student returning from a leave of absence, adding a dependent, updating financial information or major, or applying for an internship with an International Organization. Do not use this form to request an I-20 reprint (for example, if you have updated OPT employment information). For reprints, submit your request via the myGlobal portal.

**You must also submit the Financial Statement Form if applicable to your type of request.

Use this form if you are a J-1 student applying for a DS-2019 for your spouse or child

You must also submit the Financial Statement Form
Change of Status to F-1 form

Use this form if you are a current NJIT student on a different visa, and want to change your status to F-1 within the U.S. or by traveling to your home country and obtaining an F-1 visa.

Invitation Letter Use this template to write your own invitation letter for your family or friends visiting you in the U.S. You are the signatory of this letter. Please be sure to review the list of documents required by the U.S. Embassy or Consulate where your family/friend will apply for a tourist visa.
Reinstatement Form

Use this form to apply for reinstatement to F-1 status. You must meet with an OGI adviser before submitting your documents.

Request to Cancel F-2 SEVIS Record

Use this form if your F-2 dependent will no longer use their F-2 status, due to a change of status or permanent departure from the U.S.
